Job summary

Concerted Care Group in Hagerstown, MD is seeking a Licensed Mental Health Therapist to join our collaborative behavioral health team. The role focuses on building strong therapeutic relationships and delivering evidence-based treatment in our Mental Health IOP.

You will lead groups, conduct individual and family sessions, and develop trauma-informed, person-centered treatment plans for adults and adolescents, coordinating care with psychiatrists, nurses, and other clinicians.

Qualifications

• 2+ years of direct clinical experience providing mental health treatment.
• Master's degree in counseling, Social Work, Marriage & Family Therapy, or related field.
• Strong working knowledge of CBT, DBT, ACT, and trauma-informed care; EMDR experience preferred.
• Comfort using Electronic Medical Records (EMR).
• Commitment to ethical, patient-centered care and professional growth.
• Ideal candidate comfortable working with adolescents and adults.

Responsibilities

• Provide individual and group psychotherapy for adults in our Mental Health and SUD Programs.
• Work in person at our Hagerstown, MD location.
• Develop and maintain person-centered, trauma-informed treatment plans.
• Conduct comprehensive mental health assessments and level-of-care determinations.
• Educate patients on diagnoses, treatment approaches, and symptom management to support insight and empowerment.
• Design and deliver engaging psychoeducational and skills groups focused on emotional regulation, coping skills, and mental wellness.
• Collaborate with psychiatrists, nurses, PRP, and fellow therapists to provide integrated mental health care.
• Participate in treatment team meetings, case consultations, and discharge planning.
• Coordinate care with families, referral sources, and external providers as appropriate.
• Demonstrate commitment to excellent patient care, measured through outcomes like retention and program completion.
• Complete clinical documentation accurately and timely.
