Qualifications

Responsibilities

• Provide outpatient individual and family therapy using evidence-based practices.
• Conduct initial and ongoing clinical assessments in an outpatient setting.
• Assist in creating, reviewing, and updating individualized treatment plans in collaboration with clients and clinical team members.
• Ensure treatment goals are measurable, clinically appropriate, and aligned with client needs.
• Monitor client progress and document outcomes in accordance with clinical and regulatory requirements.
• Maintain accurate, timely clinical documentation in the electronic health record (EHR).
• Collaborate with care coordinators, psychiatrists, and other outpatient providers.
• Participate in clinical team meetings, case reviews, and treatment planning sessions.
• Provide psychoeducation and support to clients and families.
• Identify barriers to treatment and support client engagement in services.
• Provide referrals and coordinate care with community resources as needed.
• Adhere to ethical standards, HIPAA, and state licensure requirements.

Position Summary

The Licensed Clinical Social Worker (LCSW) provides outpatient mental health therapy services to individuals and families. This role focuses on clinical assessment, therapeutic intervention, and collaboration in the development, implementation, and ongoing review of individualized treatment plans. The LCSW works closely with clinical staff and care coordinators to ensure services are client-centered, compliant, and aligned with best practices and regulatory standards.
