Job summary

The role involves providing group and individual training on social, coping, and self-management skills, and helping patients keep appointments as part of a caring, patient-centered team.

MHT I may work in hospital, outpatient clinic, or community settings, including home, work or school, or shelters.

Qualifications

• High School Diploma or equivalent is required.
• Two (2) years of work experience and one (1) year with SPMI/Addictive Disease population or CPRP preferred in full-time status.
• Bachelor's or Master's degree in Human Services accepted in lieu of experience.
• Basic Life Support (BLS) from American Heart Association required.

Responsibilities

• Provide services to people with serious mental illness, including group/individual training on social, coping, and self-management skills.
• Support patients to keep appointments and engage in crisis prevention and management as needed.
• Work across hospital, outpatient, or community settings including home, work/school, or shelters.
• Collaborate with a multidisciplinary team to deliver compassionate, patient-centered care.
