Qualifications

• Must be at least 18 years of age.
• Valid Driver’s License.
• Some previous experience working directly with people with MH diagnosis and disabilities.
• Excellent communication skills, documentation accuracy, and organizational skills.
• Proficiency in English communication, both verbal and written.

Responsibilities

• Empower residents to make healthy choices in a safe, respectful, and supportive environment.
• Encourage residents to communicate thoughts and feelings without judgment, and help develop coping mechanisms.
• Collaborate with team and residents to build positive relationships and a natural support system in the community.
• Provide residents with knowledge to make informed decisions and offer support in choices.
• Document individual goals, progress, and barriers in daily records and service plans.
• Assist and monitor daily living skills, hygiene, safety, mobility, and use of community resources.
• Coordinate with family, guardians, medical personnel, and other team members as needed.
• Arrange and transport residents to activities, medical visits, and community outings.
• Assist residents in budgeting and managing personal monies.
