Oceans Behavioral Hospital Amarillo is seeking a Social Services Associate to support patients and families by gathering information for psychosocial assessments, discharge planning and treatment planning under supervision.

You will conduct psychoeducational groups, participate in treatment team meetings, and ensure culturally sensitive communications while aligning with Joint Commission and state regulations.

Aufgaben

• Provides social service support, education and resources for patients and families, under supervision of the Director of Social Services, per Joint Commission and state regulations.
• Plans and conducts individual or group psychoeducational sessions as assigned.
• Gathers information for Comprehensive Integrated Assessment (Biopsychosocial Assessment) within 72 hours of admission for review by a Licensed Therapist.
• Reviews Multidisciplinary assessments and provides information for Treatment Integration within 72 hours of admission.
• Performs case management and discharge planning as assigned.
• Attends treatment team meetings and reports on patient progress toward treatment goals.
• Identifies age/disability/cultural needs and communicates sensitively to patients and families.
• Performs discharge planning in accordance with needs and involves the patient’s support system.
• Performs other duties as assigned.
