Under the Director of Social Services, you will ensure timely data gathering, care coordination, and family involvement while adhering to Joint

Responsibilities

• Provide social service support, education and resources to patients and families under supervision in accordance with Joint Commission, Federal and State regulations and facility policies.
• Plans and conducts individual or group psychoeducational sessions as assigned.
• Gathers information for Comprehensive Integrated Assessment within 72 hours (or 3 sessions) of admission for licensed therapist review.
• Reviews all Multidisciplinary assessments and provides information for Multidisciplinary Treatment Integration within 72 hours of admission.
• Performs case management and discharge planning as assigned by the Director of Social Services.
• Attends treatment team meetings when indicated and reports on patient progress toward treatment goals.
• Identifies age/disability/cultural needs and communicates in a culturally sensitive manner to patients and families.
• Conducts care and discharge planning involving the patient’s support system as appropriate.
• Performs other duties and projects as assigned.
