Job summary

Community Services Group in State College, PA is hiring a Psychiatric Rehabilitation Associate for the Adult Mental Health Services program. Qualifications

• Bachelor’s degree in a field relevant to the position or an Associate’s degree with 1 year mental health direct service experience.
• Must have a valid driver’s license and be able to drive a passenger van in the community.

Responsibilities

• Coordinate and/or complete initial and ongoing assessments.
• Develop, review, and ensure implementation of individual support plans.
• Assess, evaluate, and document individual performance.
• Identify strengths and assets to help individuals achieve goals.
• Implement rehab team decisions and contribute to the team process.
• Facilitate group processes and activities.
• Transport individuals to community activities.
• Learn and model trauma-informed care principles and promote DEI.
