Job summary

Community Services Group is seeking a Mobile Psychiatric Rehabilitation Associate to support individuals in achieving their recovery goals within our Adult Mental Health Mobile program in the State College area. You will assess needs, help develop and implement support plans, and assist with transportation to community activities.

This PRN/zero scheduled hours position emphasizes trauma-informed care, cultural responsiveness, and collaboration with a multidisciplinary team.

Qualifications

• Bachelor’s degree or an Associate’s with 1 year MH direct service experience required.
• Must have a valid driver’s license and be able to drive a passenger van in the community.
• Commitment to trauma-informed care, cultural responsiveness, and rights-based practice.

Responsibilities

• Coordinate and complete initial and ongoing assessments of individuals.
• Develop, review, and ensure implementation of individual support plans.
• Assess, evaluate, and document individual performance to support recovery goals.
• Identify abilities, strengths, and assets to help achieve goals.
• Implement rehabilitation team decisions and contribute to the team process.
• Facilitate group processes and activities and transport individuals to community activities.
