Job summary

RYZE at the Ridge is recruiting a Psychiatric Rehabilitation Services Coordinator (PRSC) to provide group and individual psychiatric rehabilitative and case management services for residents with chronic mental health needs in long-term care settings. The role collaborates with an interdisciplinary team to develop and evaluate recovery-focused programs.

Qualifications

• Bachelor's degree in psychology, social work, human services or related field is required.
• Minimum of one year of supervised mental health or human services experience.
• Knowledge of behavioral health treatment and psychiatric rehabilitation principles.
• Experience with PCC or similar computerized documentation systems.
• Ability to read, write, speak and understand English.
• Ability to lift, push, pull, and move up to 25 pounds.

Responsibilities

• Provide case management and counseling services to residents as part of an assigned caseload.
• Facilitate supportive treatment interventions to maintain or enhance residents' independence.
• Provide crisis intervention and stabilization as needed.
• Coordinate with psychiatrists and other providers on treatment plans.
• Lead skills groups, psychoeducational programs and counseling sessions.
• Maintain accurate documentation per facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to support residents' goals.
• Participate in orientation and training of new staff.
