Job summary

Nexus Health Management is seeking a Psychiatric Rehabilitation Services Coordinator (PRSC) to support residents by reviewing and explaining treatment plans, and coordinating rehabilitation services. You will help residents actively participate in plan reviews and ensure ongoing engagement with services.

The PRSC role requires strong empathy, excellent communication with residents and families, and ability to document social/psychological needs.

Qualifications

• Degree in social work, psychology, sociology or counseling.
• Ability to present written reports clearly.
• Understanding of rehabilitation of mentally ill individuals.

Responsibilities

• Review and assist residents in understanding the treatment plan.
• Prepare and assist residents with active participation in the treatment plan review.
• Provide and/or coordinate the delivery of psychiatric rehabilitation services programs.
• Re-evaluate residents on a quarterly basis.
