HCA Florida Orange Park Hospital is seeking a Mental Health Professional II for PRN opportunities to provide supervised mental health services to patients and families. You will conduct assessments, educate, lead groups, and develop treatment plans using evidence-based interventions in individual, family, and group therapy.

The role emphasizes collaboration with a multidisciplinary team, discharge planning, and post-discharge follow-up to support patient outcomes in an acute care setting.

Qualifications

• Master's degree in a relevant field is required.
• Valid LPC/LMHC or LCSW or LMFT licensure or eligible licensure in Florida.

• Develops treatment plans and attends treatment planning meetings and reviews.
• Assesses environment and patient safety status and reports/responds to safety issues.
• Identifies risk factors and interventions for patients with special precautions (suicide, homicide, falls, elopement).
• Completes assessments, group notes, and documentation promptly and in compliance.
• Collaborates with the multidisciplinary team to coordinate discharge planning needs.
• Conducts post-discharge follow-up as indicated.

Job Summary And Qualifications

The Mental Health Professional II is to deliver exceptional, patient-focused care by providing supervised mental health services to patients and their loved ones. Their responsibilities include conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy.

What you will do in this Role

• Conducts one-on-one, group, and family psychoeducational and process groups.
• Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
• Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.
• Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
• Conducts post-discharge follow-up as indicated.

What qualifications you will need

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 90 days of employment start date
• Masters Degree
• (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LCSW) Clinical Social Worker, or (LMFT) Licensed Marriage and Family Therapist, or (LPC-A/MHCI) Lic Prof Counselor Assoc/Mental Health Intern, or (LBSW) Licensed Baccalaureate Social Worker, or (LMSW/RCSW-I) Lic Master Social Worker/Reg Clin SW-Intern
