Overview

The Mental Health Practitioner/Case Manager works as part of an Assertive Community Team that provides care and treatment for clients experiencing significant mental health/psychiatric symptoms, functional impairments, and possibly co-occurring chemical dependency or personality disorders. The Practitioner is a primary agent of hope and change, working in partnership with the client for recovery. Responsibilities include assessment, crisis management, planning, individual counseling, skills teaching, group work, case management, household management, financial assistance, and on-call duties.

• Effective communication skills and competency in assessment and crisis management
