Socket.dev is seeking a Housing Coordinator to support individuals with severe mental illness in securing and maintaining housing within the Supported Housing Program. The role involves coordinating with clinicians, landlords, and community partners, while ensuring program compliance and accurate documentation.

The ideal candidate will have a Social Services degree, 3+ years in behavioral health, and the ability to obtain QMHA certification within two months.

Qualifications

  • Associates or Bachelor's degree in Social Services, Psychology, Human Services, Counseling, or related discipline.
  • Three years of experience in the behavioral health field.
  • Must be able to obtain QMHA certification within two months of hire; QMHA or QMHA-R preferred.
  • Must have Oregon driver license, reliable vehicle, and insurable to travel.

Responsibilities

  • Provides direct housing support to mental health clients seeking housing and maintains accountability for ISSP goals.
  • Coordinates daily housing services and attends treatment team meetings as a mental health team member.
  • Establishes relationships with landlords and property management; develops housing opportunities.
  • Monitors housing-related expenses, completes documentation, and performs data tracking.
  • Performs outreach and client welfare checks as appropriate.
