Macon-Bibb County is seeking a Mental Health Court Case Manager (grant funded) to provide case management services for participants and help them meet program goals identified in assessments. The role emphasizes data collection, tracking progress, and facilitating access to resources while maintaining confidentiality and working with the treatment team.

The position requires an acute attention to detail, ability to work in a fast-paced environment, and occasional weekend drug screening with time

Qualifications

• Bachelor's degree preferred in Criminal Justice, Social Work, Psychology, or related field.
• Minimum of one year of case management experience and one year working with criminal justice populations.
• Experience with mental illness and addictions preferred; proficient data systems knowledge.

Responsabilités

• Provide case management services to support participants in achieving program goals.
• Communicate with Mental Health Coordinator and Treatment team regarding participant progress.
• Track participant progress, conduct individual meetings, create need-based goals, and perform data collection.
• Prepare and maintain participant files and assist clients in obtaining resources.
• Maintain confidentiality as required by federal law; administer drug screens and conduct site visits.
