NorthShore University HealthSystem in St. Charles, IL, is seeking a MH Clinical Therapist II for a full-time role with days and two evenings per week.

The clinician will perform psychosocial assessments, coordinate care, and develop tailored treatment plans. Responsibilities include individual, couples and family counseling, participation in team meetings, and maintaining thorough medical records to support discharge planning.

Qualifications

• Master's Degree in a recognized mental health field
• Knowledge of developmental psychology, disease pathology and clinical needs of assigned population
• Ability to maintain information which is highly confidential
• Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation
• Ability to demonstrate self-control in volatile or hostile situations
• Ability to set therapeutic limits and maintain therapeutic distance with clients; maintain therapeutic boundaries with patients during treatment and following termination

Responsabilités

• Conduct psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.
• Coordinate care by contacting referral sources and other providers to inform of enrollment and obtain collateral data.
• Develop, implement and monitor progress of the patient's individualized treatment plan.
• Provide individual, couples and family process-oriented counseling sessions as appropriate using various modalities.
• Participate in clinical team meetings and contribute to case discussions on psychosocial and disease management needs.
• Inform attending physicians about treatment progress and any changes in emotional/mental status affecting daily functioning.
• Transition clients to inpatient care when psychiatric hospitalization is needed.
• Demonstrate knowledge of community resources to assure follow-up care.
• Maintain accurate medical records including assessments, sessions and discharge planning notes.
