Job summary

Mahajan Therapeutics is seeking a dedicated Full Time Mental Health Case Manager to support therapists and coordinate services in our outpatient clinic. You will liaise with families, arrange transportation, and help clients access resources under supervision.

Key duties include documenting according to policy, attending hearings, coordinating ISP, crisis management, and advancing daily living skills. Requires about 1 year experience in mental health with adolescents to adults and proficiency in

Qualifications

• 1 year experience working with adolescents, teens, and adults with mental health needs.
• Proven competency with MS Office applications (Word, Excel, PowerPoint).
• Adaptability in a challenging, matrixed organizational environment.
• Satisfactory references and criminal background check; drug screen.

Responsibilities

• Act as liaison between therapist, referring agency, and families to meet client needs.
• Coordinate transportation and ensure client needs are met.
• Attend court hearings as deemed appropriate.
• Document in accordance with policy.
• Prepare reports for treatment staff and CSPT sessions.
• Coordinate ISP including services and access to community supports.
• Support crisis management and stabilization efforts.
• Assist with ongoing client assessments and daily living skills.
