Job summary

Catholic Charities Twin Cities in Saint Paul is seeking a Mental Health Case Manager I to support residents in permanent supportive housing programs. The role focuses on rehabilitative services that aim to stabilize psychiatric health, build life skills, and promote community integration.

You will conduct assessments, create trauma-informed care plans, coordinate services with providers, document progress, and help clients maintain housing stability.

Qualifications

• Bachelor's in Social Work, Psychology, Sociology, Human Services, or similar.
• Completion of a practicum or internship that requires direct interaction with adults or children served and focused on behavioral sciences or related fields.
• At least two years of supervised experience providing services to adults or children with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities.
• Experience providing licensed mental health care directives and developing treatment plans.

Responsibilities

• Conduct individual assessments of client strengths, resources and needs.
• Develop individualized, trauma-informed community support plans.
• Implement interventions identified in support plan, refer to external providers, maintain connections and provide case coordination.
• Develop therapeutic relationships with team and client, support and advocate for client in obtaining services and benefits.
• Maintain complete case documentation and ensure confidentiality and timely reporting according to DHS guidelines.
• Coordinate services internally and externally with county case managers, family/guardian, medical personnel, etc.
• Attend supervision and case review meetings and participate in team decisions.
• Maintain and update client files and treatment plans at least every six months.
• Review data for accuracy and timeliness to support accountability and outcomes.
• Provide case management to support housing retention and timely benefit renewal.
• May transport clients.
