• The position requires a bachelor’s degree in a related field and supervised experience in behavioral health.
• Experience providing services to adults or children with mental illness or substance use issues is preferred.
• Ability to work with diverse and low-income populations and maintain confidential records.

Responsibilities

• Conduct individual assessments and develop person-centered, trauma-informed plans.
• Coordinate services with external providers and support housing retention.
• Maintain case documentation and ensure DHS guideline compliance.
• Provide transportation as needed and participate in supervision meetings.
• Support client in benefits and housing-related tasks.
