Qualifications

• Bachelor’s Degree preferred.
• Degree in Human Services or related field preferred.
• Knowledge of case management concepts and ethics.

Responsibilities

• Enhance community integration by increasing opportunities for community access and living skills.
• Make collateral contacts to promote implementation of the individualized services plan.
• Assess needs and plan services including developing an ISP.
• Link individuals to community supports to meet life goals.
• Assist individuals to locate or obtain needed services and benefits.
• Coordinate services within the provider and with other agencies.
• Monitor service delivery and provide follow-up instruction and counseling.
• Advocate for individuals as their plans evolve.
• Develop crisis plans and provide emergency screening as needed.
• Plan transitions in individuals' lives and coordinate health care needs.
