Job summary
Minnesota Care Counseling Services Inc is seeking an Adult Mental Health Targeted Case Manager (TCM) to support adults with Serious and Persistent Mental Illness in Minnesota. You will meet clients in homes and community settings, coordinating assessments, plans, and referrals to promote recovery and independence.
The role requires handling functional assessments, plan development every 180 days, monthly progress reviews, and strong adherence to privacy and documentation standards in an EHR
Qualifications
- Bachelor’s degree in behavioral sciences or related field with 2,000 hours of supervised experience.
- Graduate degree in a behavioral sciences field is acceptable.
- RN with at least three years of experience serving adults with mental illness.
Responsibilities
- Conduct comprehensive functional assessments with clients and supporters, updated semi-annually.
- Develop a collaborative individual community support plan with the client, updated every 180 days.
- Refer clients to community resources and supports identified in the plan.
- Contact clients monthly to review progress toward goals and address barriers.
- Advocate for clients to access benefits and services they are eligible for.
- Maintain accurate documentation in the EHR for all contacts and plans.
- Comply with MG DPA, HIPAA, and related privacy regulations; provide notices at initiation.
- Record activities (assessments, ICSP, referrals) and ensure timely invoicing.
