Job summary

Service Access & Management, Inc. (SAM) is seeking a Mental Health Blended Case Manager to coordinate and advocate for clients within our Mental Health Service Line.

The role offers community-based work with some remote flexibility and requires compassionate, resourceful individuals who thrive in collaborative settings. Responsibilities include assessments, treatment planning, and connecting clients to services while supporting their safety and quality of life.

Qualifications

• Bachelor's degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field.
• 12 credit hours in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field AND 2 years experience.
• Strong interpersonal and communication skills.
• Comfortable using technology and working independently or as part of a team.
• Valid driver’s license and reliable transportation; ability to meet background and clearance requirements.

Responsibilities

• Conduct initial assessments to determine client needs.
• Coordinate with clients, families, and team to develop treatment plans.
• Connect clients with appropriate mental health services and resources.
• Monitor progress and provide onboarding support and encouragement.
• Respond promptly to mental health crises and ensure safety.
• Advocate for clients to access necessary resources and services.

About The Blended Case Management Role

As a Blended Case Manager within our Mental Health service line team, you'll play a vital role in coordinating and advocating for the mental health needs of the individuals served. This position is ideal for someone who is compassionate, resourceful, and thrives in both independent and collaborative work environments and thrives in fast-paced environments where collaboration matters.

Starting Rate: Based on education, experience and qualifications.

Blended Case Manager Core Responsibilities

• Conducting initial assessments to determine the needs of clients
• Coordinating with individuals served, their families, and team members to develop treatment plans
• Connect individuals with appropriate mental health services and resources that can facilitate well-being
• Regularly monitoring goal progress and providing onboarding support, guidance, and encouragement
• Respond promptly and effectively to any mental health crises that may arise, ensuring their safety and well-being
• Serve as an advocate for individuals by ensuring their rights are upheld, their voices are heard, and they have access to resources and services to improve their overall quality of life

What You Bring

Location & Schedule

This position is based in Franklin and Fulton Counties, PA. Work is primarily community-based with some remote flexibility. Office hours are Monday through Friday, 8:30 AM to 4:30 PM. Mental Health Case Managers have on-call responsibilities. Program expectations by service line: Mental Health are primarily community-based with some remote flexibility,
