Job summary

Discovery Behavioral Health in Houston, TX seeks an on-site, facility-based Care Coordination Specialist to manage the transition of client care between inpatient and outpatient settings, including Sober Living and Intensive Outpatient Programs. You will meet with clients regularly, coordinate with therapists, psychiatrists, and the treatment team, and ensure quality referrals throughout residential stays.

Qualifications

• Minimum of 3 years' experience in a mental health, chemical dependency, or healthcare setting.
• Experience documenting in hospital/mental health facilities.
• Bachelor's degree in psychology or social work; CADC certification preferred.

Responsibilities

• Develop a continuing care plan aligned with treatment team and client needs.
• Coordinate discharge planning and ensure timely documentation.
• Initiate discharge plan within 24 hours of admission.
• Coordinate care within 72 hours of admission and family contact within 48 hours.
• Facilitate 1–2 aftercare groups weekly.
• Maintain timely client sessions.
• Document outbound referrals in chart and Salesforce.
• Research quality outpatient services and sober living options.
• Build relationships with referents and community providers.
