Job summary

MADO HEALTHCARE UPTOWN is seeking a PRSC to manage multiple clients, meeting with clients and their loved ones to discuss recent successes or setbacks, and creating treatment plans for those with mental illnesses or recovering from traumatic events.

You will coordinate care with other departments, input with MDS and care plans in a timely manner, and monitor client progress while maintaining accurate records. Evening and every other weekend availability is required for this role in Chicago.

Qualifications

• Experience in coordinating care and talking with clients.

Responsibilities

• Evaluate patients based on needs, limitations and desires.
• Address patient concerns and goals with ongoing communication.
• Build rapport with patients and their families.
• Coordinate care with other departments.
• Develop care plans for patients.
• Input with MDS and care plan in a timely manner.
• Input assessment in Timely manner.
• Monitor and evaluate client progress and adjust treatment plan accordingly.
• Maintain accurate records and report on client status.
• Refer clients to community services to help in better recovery.
