Job summary

CN Guidance & Counseling Services, Inc. in Plainview, NY seeks a qualified mental health professional to join the FACT Team, delivering community-based services to individuals with severe mental illness.

You will manage records, coordinate care, and participate in quality improvement activities, both in-office and in the field, to support comprehensive case management and client outcomes.

Qualifications

• Bachelor, master's degree or higher in social work, psychology, rehabilitation counseling, or related field preferred.
• Peer counselor certification is also accepted.

Responsibilities

• Manages medical records and coordinates the MIS.
• Maintains complete case records per accepted principles.
• Leads quality improvement activities including data collection and outcome analysis.
• Plans and coordinates all services to support case management and care coordination.
• Ensures service plans reflect the recipient’s needs and are updated timely.
