Job summary

Grace Medical Center, part of LifeBridge Health, is seeking a professional to provide psychosocial rehabilitative services in an Adult Day program. You will teach ADL skills, manage client caseloads, and perform case management while serving as a liaison to community resources. Travel in a LBH vehicle may be required.

The role involves conducting individual and group ADL sessions and outreach to support client IRRPs for community integration.

Qualifications

• Provide psychosocial rehabilitative services to adults with severe and persistent mental illness in an Adult Day program.
• Must have a valid Maryland State Driver's license and clearance to operate LifeBridge Health-owned vehicles.
• Must have a valid BLS Certification from the American Heart Association.

Responsibilities

• Teach and assist members with activities of daily living skills (e.g., shopping, budgeting, food selections, safety skills, mobility).
• Maintain a caseload of clients and document on time.
• Perform case management and link to community resources; operate the LBH-owned vehicle as needed.
• Conduct one-to-one and group sessions related to teaching Activities of Daily Living skills.
• Conduct outreach in the community as indicated by the client's IRRP to support community integration.
