Job summary

Grace Medical Center, part of LifeBridge Health, is seeking a frontline professional to provide psychosocial rehabilitative services in an Adult Day program. You will teach and reinforce Activities of Daily Living skills, support community integration, and maintain a caseload with timely documentation.

Responsibilities include one-to-one and group sessions, linking clients to community resources, and required driving duties in LBH vehicles.

Qualifications

• High School / GED or equivalent required.
• Must have a valid Maryland Driver's license and be cleared by Fleet Safety.
• Must have a valid BLS Certification from the American Heart Association.

Responsibilities

• Teach and assist members with activities of daily living skills.
• Maintain a caseload of clients and complete documentation on time.
• Perform case management duties including linking to community resources.
• Operate a 7-12 passenger vehicle.
• Conduct one-to-one and group sessions related to teaching ADL skills.
• Conduct outreach with clients in the community when needed.
