Job summary

SageGrove is seeking a Memory Support Coordinator in Kansas City, MO to lead the memory care neighborhood, supervise day-to-day care, and work with the Executive Director to enhance resident well-being and teamwork.

You will oversee the Portraits Program, assist residents with daily living, coordinate activities and staff schedules, and train team members. A Montessori certification is preferred or willingness to obtain it; a valid driver’s license is required.

Qualifications

• Experience or knowledge of health-related practices for residents with dementia is preferred.
• Montessori Certification or willingness to obtain it is required.
• Must have a valid driver’s license and CPR/BLS certification.
• Strong verbal and written communication skills with HIPAA awareness.

Responsibilities

• Lead care associate in memory care neighborhood and supervise daily care under ED guidance.
• Implement Portraits Program using Montessori methods and individualized plans.
• Manage activities, schedules and staff daily work assignments.
• Perform daily housekeeping, laundry, food service as directed.
• Train staff, communicate with shifts, respond to emergencies and notify ED as needed.
• Assist with work schedules, workflows, and community marketing plans.

What You’ll Do:

As our Memory Support Coordinator, you will serve as the lead care associate in the memory support neighborhood, supervising and implementing day-to-day care under the direction of the Executive Director, directly impacting the lives of our residents and the success of our team by:

• Implementing and overseeing the Portraits Program with Montessori methods and individualized components for each memory support resident.
• Assisting residents with self-medication, personal care, and activities of daily living (including changing, dressing, bathing, and toileting).
• Developing and implementing resident activities, schedules, and staff daily work assignment sheets.
• Performing daily housekeeping and laundry duties, and assisting with food preparation, service, and cleanup as directed.
• Training staff for the memory support neighborhood and communicating daily with all shifts concerning any details about residents or activities.
• Responding to resident emergency calls and immediately notifying the Executive Director of any concerns from families or residents.
• Assisting the Executive Director in preparing work schedules, improving workflow and resident satisfaction, developing community marketing plans, and conducting tours.
• Preparing lists of suggested activities, field trips, and supplies needed, and shopping for supplies as approved.

What We’re Looking For:

• A high school diploma, GED, or high school equivalency.
• Experience and knowledge of health-related practices related to the care of persons living with dementia or cognitive impairment (preferred).
• A Montessori Certification, or the willingness to obtain it.
• A minimum age of 19, or as required by law, to provide resident transportation and/or serve alcoholic beverages at community events.
• A valid driver’s license and acceptable driving record.
• A current CPR/BLS certification.
• Good verbal and written communication skills, with the ability to effectively collaborate with diverse groups and navigate challenging situations involving emotions or conflict.
• Basic computer skills (e.g., Word, Excel) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
