BMCHS Memory Disorders Clinic seeks a Social Worker to deliver clinical counseling and support to patients with cognitive impairment and their families. The role also acts as a Patient Navigator, guiding care coordination and resources.

The position emphasizes education, advocacy, and collaboration with a multidisciplinary team. Strong interpersonal skills, MSW with MA licensure eligibility, and experience with EHRs are preferred.

Formación

• Master’s Degree in Social Work (MSW).
• Licensure or eligibility for licensure as an LCSW in Massachusetts.
• Preferred experience with geriatric or cognitively impaired populations in integrated settings.

Responsabilidades

• Provide counseling and emotional support to patients with cognitive impairments and their families.
• Coordinate care, schedule appointments, and transfer medical records across providers.
• Educate patients and caregivers about diagnoses, treatment plans, medications, and insurance navigation.
• Identify barriers to care and advocate for patient rights and resources.
• Maintain compliant clinical documentation and participate in interdisciplinary case conferences.
