Travel to Detroit, Dearborn, and West Bloomfield is required, with a fixed weekly schedule. LMSW licensure is mandatory, and the candidate should demonstrate strong communication and crisis intervention

Qualificações

• MSW degree required; LMSW licensure required.
• Experience in psychosocial assessment, counseling, and discharge planning preferred.
• Ability to identify community resources and collaborate with multidisciplinary teams.
• Strong interpersonal and negotiation skills with patients, families, and physicians.

Responsabilidades

• Create patient and family outcomes by managing psychosocial and economic co-morbidities.
• Provide education and consult on resources and discharge planning.
• Collaborate to develop and implement discharge plans and monitor progress.
• Mobilize resources to reduce risk and serve as liaison between hospital and community.
• Offer crisis interventions to patients and families.
