Job summary

Priority Life Care, LLC is seeking a Memory Care Coordinator to manage the Memory Care Unit under the supervision of the Executive Director and in collaboration with the Director of Nursing. You will hire, train, evaluate, and supervise memory care staff across shifts, while ensuring adherence to policies and quality resident care.

The role requires an LPN certification (strongly preferred) with experience in gerontological care, a clean driving record, and the ability to drive a large resident

Qualifications

• High school diploma or equivalent required.
• Associates or Bachelor’s Degree, a plus.
• LPN certification strongly preferred, Medication Tech Certification (or equivalent) required.

Responsibilities

• Manage the overall operation of the Memory Care Unit in accordance with resident needs, government regulations and company policies.
• Supervise memory care staff on all shifts, including hiring, training, evaluating and scheduling.
• Monitor patient care and conditions and review care plans daily to ensure appropriate care.
• Oversee medications, treatments, and nursing services per plans and physician directions.
• Be on-call as needed by the facility and perform related office functions (EHR).
• Foster staff development and training in collaboration with the DON and provide in-service education.
