Job summary

American Senior Communities in Lafayette, IN is seeking a Memory Care Support Specialist for Rosewalk Village. The Memory Care Center will be guided by person-centered care philosophies and evidence-based dementia practices to enhance resident quality of life.

Long-term care experience is preferred; a Bachelor’s degree in social work or a human services field is required. The role emphasizes compassionate presence, collaboration with interdisciplinary teams, and adherence to CARE values.

Qualifications

• Understanding social work principles, psychosocial assessments and interventions.
• Train, observe and redirect staff through initial education and one-on-one education of the Alzheimer disease process.
• Empathetic engagement with residents and families. Clear, effective communication with interdisciplinary teams. Conflict resolution and mediation skills.
• Creating a comforting and engaging atmosphere for our residents.
• Works with and promotes collaboration among departments, professionals, consultants, and organizations including government regulatory agencies.
• The ability to work towards a common goal of excellent care for our residents.

Responsibilities

• Manage and implement the Memory Care Center programs and standards.
• Support dementia care strategies and person-centered approaches.
• Collaborate with staff, families and external organizations.
• Provide a compassionate presence to residents and families.
• Foster teamwork and continuous quality improvement in memory care.
