Job summary

Brookdale Clarksville is seeking caring associates to provide dementia care and support to residents, following individualized service plans. You will assist with daily living activities, meals, and mobility, while promoting dignity and independence in a supportive community.

Qualifications

• High school diploma or GED; related experience preferred.
• 3–6 months related experience preferred.
• Knowledge of dementia and patient care.
• Ability to follow care plans.
• Basic English communication required.

Responsibilities

• Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents according to the individual service plan.
• Assists residents with daily living activities such as bathing, dressing, grooming, toileting, and mobility.
• Participates in team meetings and collaborates with community leadership.
• Serves meals and assists with dining in a dementia-friendly environment.
• Maintains accurate records and reports changes in residents' condition to supervisors.
