Brookdale is seeking caring individuals to provide dementia care to residents, supporting daily living activities, dignity, and engagement in a safe, home-like community. You’ll work closely with leadership to implement person-centered plans, administer meals and hydration, and document changes in residents' health and behavior.

Qualifications

• High school diploma or GED; three to six months experience in direct care with older adults preferred.
• Knowledge of dementia and person-centered care approaches.
• Ability to read, write, and communicate in English; follow written or verbal instructions.

Responsibilities

• Provide dementia care including activities of daily living per individual care plans.
• Assist with bathing, dressing, grooming, toileting, and mobility support.
• Support residents with meals, hydration, and Mobility while promoting independence.
• Record daily observations and communicate changes to supervisors.
• Maintain a clean, safe, and respectful living environment for residents.
