Brookdale Senior Living is seeking a Resident Care Coordinator for Memory Care to supervise resident care staff, coordinate training, and manage medication processes within the community. The role emphasizes delivering respectful, person-centered care and maintaining accurate documentation and schedules.

The position supports associates with a homelike memory care environment, while ensuring regulatory and Brookdale policies are followed.

Responsibilities

• Provides direct supervision of resident care staff, including monitoring job performance and coaching staff.
• Coordinates training and education of resident care staff on an ongoing basis.
• Verifies all associate hours on time cards and weekly hours report; ensures proper nursing schedules and staffing coverage per budget.
• Observes residents' health status, addresses concerns within scope of practice and reports significant changes to Health and Wellness Director.
• Assists in medication management, ordering medication, and maintaining records per Brookdale policies.
• May administer medication within scope of practice and reviews residents' capability to self-administer.
• Monitors medication administration and conducts audits of MAR.
• Communicates with families regarding residents' needs and coordinates appointments.
• Organizes and develops service plan reviews with care team and families.
• Maintains resident charts and participates in quality improvement activities.
• Performs safety and infection control checks and supports leadership on resident health and safety.
• Participates in community functions and on-call duties.
