Qualifications

• Must be at least 18 years old.
• High school diploma or equivalent.
• Minimum one year related health care experience.
• Dementia training as required by Ensemble Program.
• Positive criminal background screening.

Responsibilities

• Provide resident care services as scheduled under supervision.
• Assist with activities of daily living and mobility.
• Document care on resident service plans and records.
• Maintain daily communications with care team to ensure continuity of care.
• Promote a safe environment for residents, staff and visitors.
