Qualifications

• High school diploma or GED.
• Degree in nursing, social services, or therapeutic recreation preferred.
• Knowledge of and experience with activity program facilitation and development.
• Certification or license per state guidelines.
• Current Basic First Aid and CPR certification if required by state.
• Active Nursing License.

Responsibilities

• Lead memory care programming and Heartfelt Connections program implementation.
• Direct the Heartfelt Connections Memory Care Program, including staff training.
• Evaluate resident activity needs and collaborate with care staff to plan daily activities.
• Utilize training tools from the Orientation and Education Trainer Manual.
• Utilize the Playbook and Activities Recipe Book daily.
• Promote a holistic wellness culture across eight wellness dimensions.
• Provide education on disease prevention and management for residents and staff.
