Job summary

Avalon of Bloomfield Township is seeking a Director of Memory Care to lead and coordinate the Heartfelt Connections Memory Care Program, ensuring activities meet residents’ spiritual, emotional, intellectual, vocational, social, physical, environmental needs and personal interests. The role requires experience in program facilitation, staff training, and strong collaboration with care staff.

It also emphasizes a whole-person wellness approach and compliance with state guidelines.

Qualifications

• High school diploma or GED.
• Degree in nursing, social services, therapeutic recreation or related field preferred.
• Knowledge of and experience with program facilitation and activity program development.
• Certification or licensed per state guidelines.
• Must have current Basic First Aid and CPR certification if required by State
• Active Nursing License.

Responsibilities

• Leads memory care programming and Heartfelt Connections initiatives.
• Oversees staff training and program implementation.
• Evaluates resident activity needs and plans daily activities.
• Utilizes program tools and resources in daily routines.
• Promotes eight dimensions of wellness in resident life.
• Provides education on disease prevention and wellness programs.
