Job summary

Care Initiatives’ Ravenwood Specialty Care in Waterloo, IA is seeking a Memory Program Coordinator (CCDI) to lead memory care programming for residents with dementia and cognitive impairment.

You will collaborate with nursing, social services, therapy and activities teams, educate staff, engage families, ensure regulatory compliance, and track program outcomes to promote safety and quality of life.

Qualifications

• Experience working with residents with memory or cognitive impairment.
• Bachelor’s degree with concentration in Therapeutic Recreation, Nursing, or Social Work preferred.
• CTRS credential preferred.
• Knowledge of federal/state long-term care regulations.
• Strong commitment to empathy, respect and discretion.

Responsibilities

• Plan, implement, and evaluate memory care programming for residents with dementia and cognitive impairment.
• Provide person-centered interventions to promote engagement, independence, and quality of life.
• Collaborate with nursing, social services, therapy, and activities teams on resident care plans.
• Monitor responses to programming and adjust approaches as needed.
• Educate and support staff on dementia care best practices and communication techniques.
• Support families with education, resources, and involvement in memory program activities.
• Ensure programs and documentation meet regulatory standards.
• Track participation, outcomes, and program effectiveness through reporting.
