Memory Care Program Coordinator (Finance)

Job Summary

In alignment with Brookdale's Resident and Family Engagement philosophy, this role involves planning, coordinating, and facilitating a calendar of person-centered dementia-friendly programs tailored to meet the specific needs of each resident within the dementia care community. The coordinator provides meaningful programs that enhance residents' quality of life, recognizing individual abilities and needs, and encouraging independence while honoring residents' routines, interests, and preferences.

Essential Functions

  1. Designs a 7-day-a-week person-centered program that is engaging, creative, and meets the needs and interests of individual residents. Promotes participation across the 7 domains of well-being in a dementia-friendly environment, including sensory-focused small group activities for residents unable to participate in larger groups.
  2. Develops a monthly calendar in collaboration with residents, based on their shared interests. Ensures scheduled person-centered special events and daily programs are executed as planned.
  3. Ensures dementia care programs comply with Brookdale standards, regulations, and policies, including the BEST tool and Divisional Dementia Care Site visit requirements.
  4. Collaborates with community leadership to educate and provide resources to families and staff regarding Alzheimer's disease and other dementias. Facilitates ongoing training and in-services for staff and families.
  5. May supervise Clare Bridge Program Assistants, including hiring, coaching, performance evaluations, and corrective actions. May act in supervisory capacity in the absence of the Manager or Executive Director.
  6. Participates in planning, decorating, and executing special community events, including holidays and awareness campaigns like the Walk to End Alzheimer’s.
  7. Reports changes in residents’ health or behavior to clinical partners and collaborates on interventions.
  8. Oversees social media content to ensure alignment with Brookdale’s culture and standards.
  9. Plans and schedules programming outside the community, including driving a community vehicle if necessary.
  10. Maintains departmental budget and expenses; may manage volunteer programs.
  11. Performs personal care, housekeeping, and dietary duties as needed to maintain service quality, complying with state regulations and program standards.
