Job Summary

In alignment with Brookdale's Resident and Family Engagement philosophy, the Clare Bridge Program Coordinator plans, coordinates, and facilitates a calendar of person-centered dementia-friendly programs that meet the specific needs of each resident within the dementia care community. The role involves providing meaningful programs that enhance residents' quality of life by recognizing individual abilities and needs, encouraging independence, and honoring residents' habits, routines, interests, and preferences.

Essential Functions

  1. Designs a 7-day a week, engaging, creative, and personalized program schedule based on residents' interests.
  2. Promotes and involves residents in the seven domains of well-being through structured dementia-friendly activities, including sensory-focused small group programs.
  3. Develops a monthly calendar in collaboration with residents, ensuring scheduled special events and daily programs are executed as planned.
  4. Ensures all dementia care programs comply with Brookdale standards, regulatory requirements, and policies.
  5. Collaborates with community leadership to educate and provide resources to families and staff regarding Alzheimer’s and other dementias, including facilitating in-services and training sessions.
  6. May supervise Program Assistants, including hiring, coaching, and conducting performance evaluations.
  7. Coordinates participation in community events, holidays, and signature programs, including the Walk to End Alzheimer’s.
  8. Monitors residents' health and behavior changes, reporting and addressing concerns collaboratively with the team.
  9. Oversees social media content to ensure alignment with Brookdale’s culture and standards.
  10. Plans and schedules programming outside the community, including driving community vehicles if necessary.
  11. Manages department budget and expenses, and may oversee volunteer programs.
  12. Performs personal care, housekeeping, and dietary tasks as needed to maintain service quality, with proper training and qualifications.
