Responsibilities

Job Summary

In alignment with Brookdale's Resident and Family Engagement philosophy, plans, coordinates, and facilitates a calendar of person-centered dementia-friendly programs that meet the specific needs of each resident within the dementia care community. Provides person-centered programs that engage residents in meaningful and purposeful dementia care activities, enhancing their quality of life. Recognizes individual abilities and needs, encouraging independence, and ensuring residents' habits, routines, interests, and preferences are respected in care and programming.

Essential Functions

  • Designs a 7-day a week, person-centered program that is engaging, creative, and tailored to residents' needs and interests. Promotes and engages residents in the 7 domains of well-being programs in a dementia-friendly environment, including sensory-focused small group activities for residents unable to participate in larger groups.
  • Develops a monthly calendar in partnership with residents based on their shared interests. Ensures that person-centered special events and daily programs are scheduled and executed as planned.
  • Ensures dementia care programs comply with Brookdale Excellence Standards Tool (BEST), Divisional Dementia Care Site visit tool, state, federal, and other regulations, and meet Brookdale's mission, policies, procedures, and dementia care standards as directed by the Executive Director.
  • Collaborates with community leadership to educate and provide resources to families and staff regarding Alzheimer's disease and other dementias. This includes facilitating and coordinating dementia-focused in-services or events for families and providing training for staff, including programs like Understanding Alzheimer's & Dementia Care (UADC).
  • May supervise Clare Bridge Program Assistants, including hiring, coaching, counseling, and conducting performance evaluations. May act in a supervisory capacity in the absence of the Manager or Executive Director.
  • Works with community leadership to plan, coordinate, decorate, and participate in special events such as holidays, signature programs, Walk to End Alzheimer's, family socials, educational, and marketing events.
  • Reports changes in residents' health to clinical partners, including behavioral changes; takes appropriate actions and collaborates on interventions.
  • Oversees social media content to ensure alignment with Brookdale's culture, brand, and social media policies.
  • Plans and schedules external programming events, which may involve driving a community vehicle.
  • Maintains departmental budget and expenses, and may oversee volunteer programs.
  • Performs personal care, housekeeping, and dietary duties as needed to ensure service quality, having completed necessary training and qualifications.

Qualifications

Education and Experience

Bachelor's Degree in therapeutic recreation, gerontology, education, or a related field preferred. At least two years of direct programming experience with older adults and persons with dementia, or equivalent education and experience.

Certifications, Licenses, and Other Requirements

Must have a valid driver's license and a current 3-year driving abstract with no more than one moving violation. May be required to obtain a commercial driver's license based on community needs.

Physical Demands and Working Conditions
  • Standing, walking, using hands and fingers, reaching, climbing, stooping, kneeling, crouching, crawling, talking, hearing, and vision.
  • Ability to lift up to 50 pounds.
  • Interaction with co-workers, residents, or vendors.
  • Occasional weekend, evening, or night work as needed.
  • Possible exposure to communicable diseases, infections, chemicals, biological hazards, blood-borne pathogens, and other risks.

Must be able to travel occasionally.

Management / Decision Making

Uses limited independent judgment based on precedents and guidelines; knows when to escalate issues.

Knowledge and Skills

Basic analytic ability, understanding of the work environment, basic PC skills, and familiarity with Microsoft Windows and Word processing software.
