Job Summary

Assists the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered dementia-friendly programs that meet the specific needs of all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person-centered programs that engage residents in meaningful and purposeful dementia care activities, enhancing their quality of life. Recognizes individual abilities and needs, encouraging independence, and ensuring resident habits, routines, interests, and preferences are honored in care and programming.

Essential Functions

  1. Supports the implementation of a person-centered program tailored to individual needs and interests of residents.
  2. Assists in leading and engaging residents in the 7 domains of well-being programs—purposeful, intellectual, social, spiritual, emotional, and physical—in a dementia-friendly environment, including sensory-focused small groups for residents unable to participate in large groups.
  3. Assists the Program Manager or Coordinator in reviewing individual residents' service plans and completes program documentation as required by Brookdale standards or state regulations.
  4. Helps ensure dementia care programs comply with state, federal, and other regulations, aligning with Brookdale's mission, policies, procedures, and standards.
  5. Provides guidance and resources to staff on the importance of programming and engagement to improve residents' quality of life.
  6. Coordinates and participates in community events such as family socials, marketing, and open houses.
  7. Communicates positively with residents, families, and community professionals about the dementia care program and its benefits, and with families regarding residents' personal needs and successes.
  8. Assists in meeting the department’s quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
  9. Supports planning and scheduling of programming events outside the community, which may include driving a community vehicle.
  10. Assists with volunteer programs, including training and supervision of volunteers.
  11. Performs personal care, housekeeping, and dietary duties as needed to maintain service quality per Clare Bridge Program standards.
