You will evaluate and develop individualized plans, address gait, balance, transfers, and mobility, and partner with caregivers to support participation and dignity.

Responsibilities

• Evaluate and develop individualized plans of care
• Address gait, balance, transfers, strength, endurance, mobility, and fall risk
• Practice functional mobility in the environments where residents actually live
• Incorporate real-world routines and meaningful activities into treatment
• Adapt interventions based on cognition, communication, behavior, and environment
• Identify environmental barriers and collaborate on solutions
• Partner with caregivers and community staff to support carryover
• Help residents maintain mobility, participation, dignity, and quality of life
• Build relationships with residents and learn the person behind the diagnosis
