Job summary

Goodworks-Unlimited is seeking a Memory Care Nurse to provide compassionate, high-quality nursing care to residents with dementia in our memory care community in Franklin, TN.

You will lead care teams, perform assessments, manage medications per physician orders, document thoroughly, and coordinate with families and other healthcare professionals to promote dignity, safety, and comfort for residents.

Qualifications

• Current LPN/LVN license as required by the state.
• Experience with older adults or memory care preferred.
• Knowledge of dementia care preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and as part of a team.
• Calm and professional during emergencies.

Responsibilities

• Provide direct nursing care to residents according to their care plans.
• Monitor residents for changes in physical, mental, behavioral, and emotional condition.
• Complete nursing assessments and report significant changes to the physician or care team.
• Maintain accurate nursing documentation and communicate concerns to physicians/families.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide education and training regarding dementia care and safety to staff.
