Qualifications

Responsibilities

  • Coordinate memory care admissions with the Wellness Director to ensure accommodations meet resident needs.
  • Develop, create, plan, and organize the daily program and events; ensure 24/7 implementation with Life Enrichment Director support.
  • Monitor memory care staff performance to ensure safe, compliant care.
  • Oversee medication systems in line with state and Galerie Management guidelines.
  • Conduct resident history and preferences assessments; develop individualized service and program plans.
  • Coordinate care planning with families, physicians, and home health agencies.
  • Provide training and orientation to new staff; cover staffing as needed.

Job Title: Memory Care DirectorReports To: Wellness Director/Executive DirectorSummary: The Memory Care Director works as a liaison between residents, resident families, and staff. They will develop, coordinate and implement programs and services for residents with cognitive impairment; to provide a therapeutic environment that maintains each resident's highest level of wellbeing.Responsibilities and Duties:Ensure that the policies and procedures are effective and enforced to support the health and safety of the residentsCoordinate memory care admissions with the Wellness Director including supervising move ins to be sure accommodations are as desired, assessments are completed as necessary, and care is immediately implementedDevelop, create, plan, and organize the daily program including the printed calendar of events and ensure its implementation 24 hours per day, 7 days per week with assistance from the Life Enrichment DirectorMonitor and document memory care staff performance to ensure that care and services are being delivered safely and in accordance with the state regulations and Galerie Management standardsManage and supervise memory care associates by providing assignments, support, direction, feedback, and trainingPartner with and otherwise assist the Wellness Director in establishing and maintaining a medication system which adheres to state, federal laws, regulations, and Galerie Management guidelinesNotify Wellness Director of at-risk residents in need of a higher level of care. Conduct individual resident history, preferences assessments, and develop individualized service and program plansCoordinate care planning with families, physicians, and home health agencies while working within the Community’s policies and proceduresWork together with the Wellness Team in scheduling and supervising staff; be available to cover staffing when needed.Work together with the Wellness Team in providing training and orientation to new assisted living staff members and ongoing training to current staff membersFacilitate a pro-active problem-solving approach to challenging resident behaviorsMay drive community vehicle for medical, social, or other outingsMay participate in Manager on Duty Program.This job description represents an overview of the responsibilities for the above referenced position.
