Job summary

Living Branches in Souderton, PA is seeking a LPN/RN for our New Memory Care Unit. A full-time position is available, with part-time shifts also offered. You’ll build relationships with residents, implement care plans, and support a welcoming, community-focused environment.

We value licensure, HIPAA knowledge, and dedication to person-centered care. This role emphasizes teamwork, emergency response, and collaboration with physicians and families to enhance residents’ well‑being.

Qualifications

• Valid LPN/RN License
• 2-4 years of related experience (Preferred)
• Knowledge of HIPAA rules and regulations and guidelines governing personal care and/or long‑term care

Responsibilities

• Utilize the nursing process to evaluate resident needs and develop care plans
• Administer nursing care and evaluate outcomes of care
• Record vital signs including temperature, pulse, and blood pressure
• Respond to emergency calls from residents and notify physicians or supervisor
• Maintain compliance with regulations and documentation standards

LPN/RN Overview:

• Utilize the nursing process, procedures and techniques to perform various duties such as evaluating resident needs, developing individual care plans, administering nursing care, and evaluating nursing care
• Ensures that temperature, pulse, blood pressure and other vital sign checks including pacemakers checks are completed and recorded
• Responds and reacts to emergency calls from residents.
• Notifies physicians, supervisor, family or other appropriate personnel when unusual resident condition exists.
• Valid LPN/RN License
• 2-4 years of related experience (Preferred)
• Ability to handle multiple responsibilities simultaneously, has ability to prioritize work and address emergencies as they may occur
• Knowledge of HIPPA rules and regulations and of regulations and guidelines governing personal care and/or long‑term care
