Job summary

Luxor Healthcare in Missouri is seeking an experienced Memory Care LPN to deliver compassionate, person-centered nursing care for residents living with dementia.

Responsibilities include implementing individualized care plans, assisting with ADLs, monitoring changes, administering medications, documenting observations, and collaborating with RNs and the care team. We offer flexible scheduling, competitive pay, PTO, benefits, and opportunities for growth.

Qualifications

• Current LPN license in Missouri required.
• Experience in memory care or dementia care preferred.
• Strong communication, clinical, organizational and leadership skills.
• Compassionate, patient-centered approach and team player.

Responsibilities

• Deliver high-quality, compassionate nursing care to residents per individualized care plans.
• Provide person-centered care to residents with dementia and memory-related conditions.
• Assist with activities of daily living while promoting dignity and independence.
• Promote resident engagement and emotional well-being through meaningful activities.
• Create a safe, secure environment and follow nursing processes.
• Monitor residents and document vital signs, observations, and changes.
• Administer medications and treatments as ordered per policies.
• Communicate with RNs, physicians, CNAs, families and the care team.
• Provide guidance to the nursing staff to ensure high-quality care.
