Job summary

Trilogy Health Services is seeking a Memory Care Leader (CNA) for Violet Springs Health Campus in Pickerington, OH. The role oversees Legacy Lane, ensuring dementia care practices support resident wellbeing and guides families through the care continuum.

You'll educate campus staff, lead dementia initiatives, interview candidates, and maintain a dementia-safe environment through daily rounds and collaborative leadership with the Executive Director.

Qualifications

• High school diploma or GED required.
• Current valid state CNA certification required.
• 1–3 years of relevant memory care experience preferred.

Responsibilities

• Oversee daily operations of the Legacy Lane programs on the health campus, ensuring alignment with dementia care best practices.
• Lead staff education initiatives on dementia care topics and act as the Dementia Champion for the campus.
• Assist in maintaining resident population per memory care move-in/move-out criteria.
• Develop personalized Service Plans for memory care residents and guide families through the care continuum.
• Maintain open communication with campus staffing to ensure specialized dementia care staffing.
• Participate in interviews for Legacy team members, assessing skills and compassion for residents with dementia.
• Ensure a dementia-safe environment by daily rounds in resident rooms and the Legacy Lane area.
