Job summary

RM Management is seeking a Memory Care Home Health Aide to provide compassionate, personalized care for residents with memory loss. You will support independence, dignity, and quality of life through dementia-centered approaches and daily activities.

As part of our memory care team, you’ll follow state regulations, document care details, and collaborate with nurses and leadership to ensure consistent, high-quality care for our residents.

Qualifications

• High school diploma or equivalent is required.
• CNA/NAR or Home Health Aide certification is preferred.
• Compassionate, patient, and respectful with dementia care.
• Strong communication, listening, and problem-solving skills.
• Ability to work independently and as part of a team.
• Ability to follow care plans, medication instructions, and RN delegation.
• Ability to read, understand, and complete documentation in English.
• Commitment to resident confidentiality, dignity, safety, and rights.
• Ability to lift and carry approximately 30 pounds or more.
• Willingness to complete required training and ongoing education.

Responsibilities

• Provide personal care (bathing, dressing, grooming, toileting, mobility) and ADLs.
• Administer medications per MAR and RN delegation.
• Build meaningful relationships with residents through patience and consistency.
• Use person-centered dementia care approaches for memory loss and behavior changes.
• Monitor changes in residents and respond with redirection and reassurance.
• Encourage participation in activities and daily routines.
• Create positive experiences through daily interactions.
• Maintain a clean, safe living environment.
• Complete required documentation and reports per state policies.
• Communicate changes in resident condition to RN or leadership.
• Follow infection control principles and resident rights policies.
• Collaborate with team to ensure high-quality care.
• Maintain reliable and punctual attendance.
