Job summary

Evergreen Senior Living in Decatur, IL seeks a licensed practical nurse to lead our memory care program. The Memory Care Director will oversee daily operations, mentor CNAs, and ensure compassionate, resident-centered care for residents with Alzheimer’s and other dementias.

Qualifications

• Licensed Practical Nurse with leadership experience in dementia care.
• Minimum 2 years of nursing experience.
• Experience in assisted living, memory care, or long-term care preferred.
• Strong knowledge of Alzheimer’s/dementia care and resident-centered services.
• Proficiency with EHR documentation.

Responsibilities

• Lead and oversee daily operations of the memory care community.
• Supervise, coach, and support CNAs and nursing staff.
• Conduct resident assessments, care plan reviews, and service plan meetings.
• Partner with residents, families, physicians, and interdisciplinary team members to ensure quality care.
• Develop and implement dementia-focused programming and activities.
• Monitor resident wellbeing through rounds and evaluations.
• Coordinate staffing, scheduling, training, and performance management.
• Provide education and guidance on dementia care best practices.
• Foster a safe, welcoming, and dignified environment for residents.
